ELIGIBILITY: Based on frailty, 60 years and older,
etc. - no income eligibility
APPLICATION PROCEDURE: All referrals are prescreened
over the phone to determine if they meet program criteria.
All referral sources are told at that time whether
or not client/family member/etc. is eligible. Anyone
can make a referral by calling (810)388-0096.
CAN HELP BE IMMEDIATE: No. In-home assessments are
scheduled and a care plan developed which indicates
service needs. Then services are arranged/purchased
and monitored.

PROGRAM/SERVICES DESCRIPTION: Community Care Management
(CCM) is designed to help the frail elderly to continue
living independently by arranging for supportive health
and human services in the home. CCM uses a professional
nurse and social work team to provide a comprehensive
in-home assessment, a personalized care plan that
maximizes the older person's independence and to arrange
appropriate health and human services/in-home equipment
required under the care plan. Ongoing follow-up and
monitoring continue as long as the client needs CCM.
Also, an In-Home Service program and a Medicaid Waiver
Program. The Medicaid Waiver Program is open to older
adults and younger persons with disabilities who have
limited financial resources. Requester must meet medical
eligibility for nursing home admission.
Also, have a short term, immediate response program,
consumer must be 60 years old, received discharge from
health care facility or re-hab, or urgent caregivers
situation and require personal care assistance.
Out of home services are available to clients or caregivers
over age 60.
